To gain copies and/or information on a person's birth or death record, the following apply:

1. A minor child’s birth record can be obtained by parents or grandparents only, with proper identification. Any other individual must have a notarized statement from the parent(s) giving that person permission to pick up the birth record.

2. A death record is public record and may be picked up by anyone.

3. NOTE:  EFFECTIVE JULY 1, 2010
The fee for a birth/death record is $25.00 for the first certified copy, per individual.  Each additional certifed copy is $5.00 each, per individual.
